Position Summary: The primary function of this position is to conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines standards and regulations which govern the facility, and, as directed by the Director of Nursing services and/or the Administrator so as to ensure each resident’s individual needs are identified and addressed accordingly.

Educational and Experience Requirements

• Must possess a current and valid license issued by the Commonwealth of PA.
• Prior long-term care experience
• Minimum of two (2) years’ experience with the RAI process
• Working knowledge of MDS 3.0
• Previous case management experience
• Previous clinical liaison experience preferred
• Current certification in cardiopulmonary resuscitation.
• Ability to read writes and speaks English.
• Ability to follow oral and written direction.
